If you are travelling abroad you may require vaccinations.  A holiday vaccination form needs to be completed and handed in to the Surgery.  Maria our Nurse will look into the areas you are visiting and will determine whether you need vaccinations or not.  You will need to leave 5 working days before you ring in and enquire as to whether an appointment needs to be made for vaccinations. It is important to complete this form as early as possible at least 6 weeks before travelling.   If you do need vaccinations an appointment needs to be made 3-4 weeks before your departure.  Please do not leave this until the last minute as we may be unable to fit an appointment into an already full surgery.
