New Patient Registration

registrationTo register please contact our reception to fill in the appropriate forms.

All new patients are required to complete a new patient questionnaire and ethnic origin form upon registration.

You will then be given an appointment with the Health Care Assistant or Nurse for a new patient check. 

In order to prevent fraudulent registrations, all new patients registering with this Practice will be asked to provide proof of identity and residency at the point of registration.  This should be one document each from the following lists:

Proof of ID

Birth Certificate

Marriage Certificate

Passport

Photo ID Driving License

HM Forces ID Card

Proof of Residency

Utility bill e.g. gas, electricity, water, telephone (must be within 3 months of current date)

Bank/Credit Card Statement (within 3 months of current date)

Mortgage Statement (within 3 months of current date)

Current Benefits Book (within 12 months of current date)

Council Tax Bill (for current billing period)

If you do not produce the relevant documents at the point of registration then you will be asked to produce them within 4 weeks.  Please be aware that you will not be registered with the practice until appropriate identification documents have been seen and you have attended a new patient check with the nurse.

If you are registering a child under the age of 5 years then we would ask that you bring their Child Health Record Book (baby book) with you at the point of registrations.  This is to ensure that your child is up to date with their immunisations.
